The whole monkey case for klotho longevity shots rests on one study. A single low dose of klotho protein improved working memory in old rhesus monkeys (Study). What Klotho Actually Is
Klotho is a protein your body makes, mostly in the kidney. It is named after the Greek fate who spins the thread of life. Most of it stays fixed to the outside of cells. A piece can break off and travel in the blood, and that loose form is called soluble klotho.
Its day job is plumbing. Klotho helps the kidney handle phosphate, and it helps switch on vitamin D (Review). It also acts like a hormone. Its signals reach organs far from the kidney, including the brain (Review).
Blood levels tend to drop with age, though the link is loose (Study). In one set of reference values, adults aged 55 to 85 ran well below adults under 35 (Study). Levels also fall in long-term kidney disease (Review).
Low klotho is linked to a higher chance of dying from any cause (Review). That is a link, not a cause. Sick kidneys lower klotho, and sick kidneys shorten life on their own. So it is hard to say which one drives the other.
The same problem runs through the whole field. Almost everything we know about klotho in people comes from watching blood levels (Review). Nobody has changed those levels in a person and tracked what happened. The studies that did change klotho were done in animals.
The Mice That Started This
The name came from a mouse. In 1997 researchers broke the klotho gene in mice before birth. Those mice died young and could not breed. They also got hardened arteries, thin skin, weak bones and damaged lungs (Study). The paper said it looked like human aging.
The speed is the striking part. These mice grew normally for three or four weeks, then stopped growing and fell apart. They lived about two months, against two to three years for a normal lab mouse (Review).
Then came the opposite experiment. In 2005 a team bred mice that made extra klotho in every tissue from conception. They tested two lines of mice. Males lived about 20% longer than normal mice in one line, and about 31% longer in the other. Females gained about 19% (Study). Those are average lifespans. The biggest number came from one line in one sex.
The method matters here. Both experiments changed the gene in the embryo. Neither one gave klotho to a grown animal, and a shot sold to a 55-year-old is a different thing.
One Shot in Aged Monkeys
This is the study every seller points at. Researchers gave 18 old rhesus monkeys one injection of klotho protein under the skin (Study). The monkeys were 15 to 28 years old, about 22 on average, which is roughly 65 in human terms.
Then the animals did a memory task. They watch food get hidden in one of several wells, wait, then choose. The task tests working memory, the short-term space you use to hold a phone number in your head. Scores improved within four hours of the shot. On the harder version, the gain was still there about two weeks later (Study).
Two details usually get left out.
First, the team tested three doses, and only the smallest one worked. In a side part of the same study, only a handful of monkeys got each higher dose. Neither higher dose produced a clear change (Study). More was not better. The authors think a lower, more natural level may be the useful one.
Second, klotho does not seem to reach the brain. The same group injected klotho into the body and found none of it in brain tissue (Study). Something else seems to carry the message. Klotho stirs up platelets, the tiny cells that help blood clot. It raises blood levels of one substance they release, platelet factor 4. Injected on its own, that substance did reach the brain, and it improved memory in aging mice. In young mice, two common drugs that block platelets also blocked klotho’s memory benefit. That points to platelets as the go-between.
That may explain the odd timing. Klotho itself cleared out fast. The team estimated that half of it left the blood within about 30 hours (Study). That estimate rests on samples from just two monkeys. The memory gain lasted two full weeks.
Now the limits. Only about nine of the monkeys actually got the dose that worked. The team also declares its own business interests. They include a patent on klotho for thinking skills. Two of the authors founded a company to develop klotho treatments (Study). That is normal in this field, and worth knowing. One small monkey study is as far as this evidence goes.
Where Klotho Longevity Evidence Stops
There is human data. It comes from genetics, not from treatment.
Some people carry one copy of a klotho gene variant called KL-VS. In 2014 a study of 718 older adults without dementia found that carriers scored better on thinking tests (Study). About a quarter of that group were carriers. The result started much of the excitement, though the difference itself was small.
The same paper carries a warning. People with two copies got no benefit at all. The authors say a second copy wiped out the advantage, but they do not show that data. A gene that helps in one copy and not in two is not a simple lever.
Bigger groups then failed to find the benefit. A Danish study of 1,480 people aged 92 to 100 found no advantage (Study). Carriers scored slightly worse on a standard memory screen. Even the 2014 finding would not justify a shot. Carrying a gene variant from birth is one thing. Topping up a protein in middle age is another. And no klotho therapy has been given to people for memory or lifespan (Review).
What the Clinics Actually Sell
The product is not a supplement. It is gene therapy. You get an injection of genetic instructions, so your own cells make more klotho.
Minicircle sells its follistatin gene therapy at a clinic in Roatan, Honduras, for a reported $25,000 a shot (Report). It is not approved in the United States. Reporting on the trade also names clinics in Mexico, Panama and the Bahamas. One of them, in Mexico, sells klotho gene therapy alongside it (Report).
Delivery differs by company. Minicircle uses a plasmid, a small ring of DNA, injected into belly fat (Registry). A competitor plans to deliver its follistatin product inside a modified virus (Report).
The only registered human klotho studies are run by the companies selling the therapy.
Two Minicircle pilots sit on the public registry. The first gave klotho gene therapy to 14 people (Registry). The second gave klotho plus follistatin to 14 more (Registry). Neither study had a comparison group, and everyone knew who got what. Neither had an outside committee watching the safety data.
Both records say the product is given outside the United States, beyond the reach of the FDA. The main things measured are your blood klotho level and side effects you report yourself. The combined pilot finished in April 2026 and still has no posted results.
A third study, from Klothea Bio, plans 21 people at the same clinic in Honduras. Two thirds would get klotho mRNA and one third a dummy injection, with nobody told which (Registry). That design is a real improvement. But the registry record, posted in April 2026, still reads “not yet recruiting”. The Number They Sell You
A klotho blood test sounds like the responsible first step. It is not a number you can act on yet.
Labs measure soluble klotho in two main ways, and the two disagree. Researchers ran 112 blood samples through both, none of them ever frozen (Study). The results barely lined up. Among the 77 people in that trial who also had kidney function measured, the research method tracked kidney function closely. The commercial kit did not track it at all.
Reference ranges are shaky too. A widely used set comes from just 346 healthy adults, and its normal range runs from about 340 to 1,670 (Study). The top of normal is roughly five times the bottom. A larger group of 890 adults was measured with the same kit, and their middle values sat near 650 to 680 (Study). In that group, klotho tracked age and kidney function only weakly.
There is also no target. Nobody has shown that moving your klotho number changes your memory, your health or your lifespan. It is not even clear whether blood klotho reflects the klotho around your brain. For years the two were assumed to be separate. A biological age test has the same problem: a precise-looking number with nothing proven attached to it.
Compare that with a kidney test. Kidney function has agreed ranges, a clear meaning and treatments that change what happens to you. That is what a number you can act on looks like.
What You Are Really Risking
Nobody has published safety data for klotho gene therapy in healthy people. The two finished pilots have posted nothing (Registry).
Some sellers deliver the gene inside a modified virus. That method has a known downside at high doses. In one study, 38 people got a virus-delivered gene therapy into the bloodstream (Study). Everyone on the standard steroid plan showed an immune reaction. Twenty of them had regular blood smears, and six of those developed signs of damage to small blood vessels. It showed up one to three weeks after the dose.
A group given two extra immune-calming drugs had almost none of that reaction. But nobody flipped a coin to decide who got what. The groups also differed in age, dose and product. All of it happened inside a supervised trial, with monitoring and a hospital able to respond. A clinic visit abroad comes with neither.
The wider record on medical tourism is not reassuring. One review of unproven cell treatments collected 35 published cases of serious harm or death, from clinics in 14 countries (Review). Reported harms included infections, tumors, heart events and vision loss. Most patients who took legal action ended up settling, with no court ruling.
